Summer Camp '08 Enrollment Form

1. I agree to pay the full tuition balance by May 30, 2008 (a $50 deposit is due at the time of enrollment). Enrollment after May 30, 2008 requires full payment at time of enrollment.
2. BISNY Summer Camp has permission for my child to participate in all camp programs, including field trips that are planned and supervised by BISNY Summer Camp.
3. BISNY Summer Camp has the unrestricted right to terminate this enrollment agreement at its sole discretion. In the event of such termination due to camper behavior, BISNY is not obligated to refund tuition or any unused amount of the tuition.
4. CANCELLATION POLICY: If canceling prior to May 30, 2008, tuition is fully refundable. After May 2008, tuition is refundable at the sole discretion of BISNY Summer Camp.
5. BISNY Summer Camp has permission to reproduce and publish any photograph, video or likeness of my child for advertising, commercial or any lawful purpose.
6. BISNY Summer Camp has permission to treat my child for routine, minor injuries such as scrapes and bruises. In the event that a parent, emergency contact or the family physician cannot be contacted in an emergency, BISNY Summer Camp has the permission to my child examined at a hospital emergency room.

WAIVER AND RELEASE
Acknowledging that participation in athletics carries with it a risk of physical injury, I agree that The British International School of NY, its agents and employees shall not be liable to me or my child for any injury or damage, howsoever caused, resulting directly or indirectly from my child’s participation in the British International School Summer Camp at any time preceding, during or after camp is in session and I hereby discharge The British International School of NY from all actions, claims, and demands I or my child may have for any such injury or damage. I authorize that The British International School of NY has the right to use all photographs or videos taken of my child during camp for advertising or promotional material.
